The Outcome: The class investigated for root cause using various tools learned in class: brainstorming; cause-and-effect diagram; and check sheet to determine that: set-up and start-up losses were not factored into material usage calculations, resulting in the inventory inaccuracy and also resulting in incorrect standard costs. These were corrected and inventory accuracy improved to 99%, saving $6400 in annual expediting expense plus the labor for recounts.
