Ten Ten Ten Announces Maximizing Quality Performance, a Value-Based Care Implementation Guide

New guide helps Independent Primary Care practices turn quality measures into daily work, from planning the year to closing care gaps

PITTSBURGH - EntSun -- Ten Ten Ten, a nonprofit dedicated to improving U.S. health outcomes and reducing healthcare costs, today announced the release of Maximizing Quality Performance, a value-based care implementation guide for independent primary care practices.

The 81-page guide helps practices move quality performance from a year-end scramble to a year-round discipline. It covers contract requirements, provider scorecards, electronic medical record quality modules, patient outreach, Annual Wellness Visits, patient experience, and health disparities. Tools include a quality measures cheat sheet, an outreach calendar, and a training framework for medical assistants and nurses.

"A quality gap gets closed because someone knows what the patient needs and takes responsibility for the next step," said Jenn Block, PhD, MBA, Ten Ten Ten cofounder and Chief Architect, Value-Based Care. "That starts before the visit, with a prepared team, clear roles, and the right information. This guide helps practices build those routines."

The guide begins with work that should happen before January 1: understanding each contract's measures, thresholds, and documentation requirements. It shows how to bring care gaps into scheduling, rooming, and follow-up, including an approach for practices without an integrated quality module.

For patients who do not visit the office, it outlines at-home testing, home-based wellness visits, and community outreach. A reusable Preventive History List carries screening dates, results, and next steps forward between Annual Wellness Visits.

The guide also addresses appointment access, communication, and follow-through. Its health disparities section helps practices identify overlooked patients and track follow-up after abnormal screening results.

"Meeting a quality measure does not mean the patient's care is complete," said Bhargav Raman, MD, MBA, Ten Ten Ten cofounder and Lead, Clinical Strategy & Analytics. "A screening can satisfy a measure while an abnormal result still needs follow-up. Practices need systems that address both."

Lead authors Jenn Block and Bhargav Raman developed the guide with contributions from Valerie Choniuk, MPA-HCA, CPXP; Kate Eisenberg, MD, PhD; and Faarina Khan, MD, MPH.
