OpenTelemed Launches "Care Without Addresses" Initiative to Bring Telehealth to Homeless

KALISPELL, Mont. - EntSun -- How the Program Works:
1.  Technology in Trusted Spaces: Telehealth stations are installed in partner shelters and service centers. Outreach workers carry secure tablets to engage individuals on the street or in encampments.
2.  Trauma-Informed, Specialized Care:** Patients are connected to clinicians trained in trauma-informed care, substance use disorders, wound care, and psychiatry.
3.  Simplified Access: A streamlined intake process removes traditional barriers; no ID or permanent address is required to receive care.
4.  Wraparound Support: The model integrates with case management to connect patients with prescriptions (via partner pharmacies), social services, and future follow-up care, creating a continuum of support.

Addressing a Critical Need:
Individuals experiencing homelessness face disproportionately high rates of chronic illness, mental health conditions, and acute infections, yet they are among the least likely to access traditional clinic-based care. Emergency departments often become their only point of contact with the healthcare system, leading to worse outcomes and higher costs.
